The information we collect depends on your interaction with us and the services we provide.

  • telephone, mobile and email contact details;

  • information relevant to providing denture and dental prosthetic services;

  • payment and transaction information; and

  • other information reasonably necessary to provide our services.

Some of the information we collect is health information, which is considered sensitive information under Australian privacy law.

We aim to collect only information that is reasonably necessary for providing our services and managing our practice.

3. How do we collect your information?

We generally collect personal information directly from you.

This may occur when you:

  • contact us by telephone or email;

  • submit an enquiry through our website;

  • make an appointment;

  • use our online booking system;

  • attend our clinic;

  • complete a patient or medical history form;

  • communicate with one of our dental prosthetists;

Where appropriate and authorised, we may also receive information from another health professional involved in your care, such as a dentist or doctor.

  • providing denture and dental prosthetic services;

  • planning and providing treatment;

  • maintaining accurate clinical and patient records;

  • communicating with you about appointments and treatment;

  • arranging and managing appointments;

  • sending appointment reminders by SMS or email;

  • communicating with you about matters relating to your care;

  • communicating with other health professionals involved in your care, where appropriate;

  • complying with legal, professional and regulatory obligations;

  • managing and improving our practice and services;

We do not use your personal or health information for direct marketing purposes.

5. Appointment reminders and communications

We may use your telephone number, mobile number and email address to communicate with you about your appointments and treatment.

This may include appointment confirmations, reminders, changes or cancellations, treatment-related information and other administrative communications.

These communications are for the management and delivery of your healthcare and are not intended to be marketing communications.

6. Praktika — patient records, forms, bookings and SMS

Middleton Road Denture Clinic uses Praktika as our practice-management system.

Praktika is used for:

  • maintaining patient records;

  • managing appointments;

  • online patient bookings;

  • sending patient history and other electronic forms;

  • sending SMS appointment reminders and other appointment-related communications; and

  • managing information associated with the provision of our services.

Praktika advises that its servers and data centres are located entirely within Australia and that patient personal and health information stored within Praktika is not stored or transmitted to servers outside Australia.

Based on this information, patient information held within our Praktika system is stored within Australia.

Praktika may also send emails to patients on our behalf as part of the management of their care, including emails containing or linking to patient history forms, appointment-related information and other communications associated with the management of a patient's care.

7. Email communications

Our clinic also maintains email accounts through Google/Gmail.

The email address published on our website may be used by prospective or existing patients to contact us. For example, you may use this email address to make an enquiry or request an appointment.

When you contact us through our website email address, your email address and the information contained in your email may be processed through Google's email services.

We do not routinely use our Gmail accounts to send patient appointment reminders or patient history forms. Those communications are handled through Praktika.

Because Google operates services and infrastructure in multiple countries, information contained in emails sent to or received through our Gmail accounts may potentially be processed or stored outside Australia.

We take reasonable steps to protect personal and health information contained in email communications.

8. Photographs and clinical images

From time to time, we may take photographs or other clinical images of a patient, including images of the mouth, teeth, dentures or oral condition.

Clinical photographs may be taken where reasonably necessary for assessment, treatment planning, monitoring treatment or maintaining an accurate clinical record.

Where a photograph is taken for a purpose other than providing or documenting healthcare — such as advertising, social media, promotional material, education or publication — we will obtain appropriate consent before using the photograph for that purpose.

We will not use patient photographs for advertising, social media or promotional purposes without appropriate consent.

Clinical photographs are treated as health information and will be stored and protected in accordance with our privacy and information-security obligations.

9. How we use and disclose your information

We will generally use or disclose your personal information for the primary purpose for which it was collected, including providing and managing your healthcare.

Where appropriate, we may disclose relevant information to:

  • other dental practitioners or health professionals involved in your care;

  • dental laboratories involved in the manufacture or repair of dentures or dental appliances;

  • service providers who assist us in operating our practice;

  • our practice-management and online booking provider;

  • SMS, email or communication service providers used to communicate with you;

  • payment processors and financial service providers;

  • insurers, professional advisers or legal advisers where reasonably necessary;

  • government agencies, regulatory bodies or other organisations where required or authorised by law; and

  • other persons where you have provided appropriate consent.

We will limit disclosures to information reasonably necessary for the relevant purpose.

We will not sell your personal information.

10. Overseas storage and disclosure

Personal and health information held within our Praktika practice-management system, including information associated with patient history forms, online bookings and SMS communications, is stored in Australia based on information provided to us by Praktika.

Our separate Gmail email services may involve personal information being processed or stored outside Australia.

Where personal information is disclosed to an overseas recipient, Middleton Road Denture Clinic will take reasonable steps to comply with its obligations under the Privacy Act 1988 (Cth) and the Australian Privacy Principles, including Australian Privacy Principle 8 concerning cross-border disclosure of personal information.

We will take reasonable steps to ensure that personal information is handled securely and appropriately by the service providers we use.

11. Information security

We take reasonable steps to protect the personal information we hold from:

  • misuse;

  • interference;

  • loss;

  • unauthorised access;

  • unauthorised modification; and

  • unauthorised disclosure.

Security measures may include appropriate physical, electronic and administrative safeguards.

Access to patient information is limited to people who require the information to perform their duties or provide services.

However, no method of storing or transmitting information electronically can be guaranteed to be completely secure.

12. How long do we keep your information?

We retain personal and health information for as long as reasonably necessary for the purposes for which it was collected, including to provide ongoing care, maintain appropriate clinical records and comply with applicable legal, professional and regulatory requirements.

When information is no longer required, we will take reasonable steps to securely destroy or de-identify it, subject to applicable legal or professional record-keeping requirements.

13. Access to your information

You generally have the right to request access to personal information we hold about you.

If you would like to access your patient or health records, please contact us using the details provided in this policy.

We will respond to your request within a reasonable period and in accordance with applicable privacy laws.

In some circumstances, we may be permitted to refuse access to information. If this occurs, we will explain the reason for the refusal where we are legally able to do so.

14. Correcting your information
